Incident Reporting Forms

There are several options and types of reports you can make at Jacksonville State University, including Concerning Behaviors, Conduct, Hazing, and Title IX. Please review the information on each report form below.

Please use the Concerning Behavior Report Form to report students, faculty, or staff who exhibit distress, threatening, or erratic behavior; individuals in distress; or those presenting behavioral or safety concerns.

Please use this Conduct Incident Report Form to report student(s) and/or student organizations that were involved in an incident with prohibited conduct.

Please use this Hazing Incident Report Form to report any suspicions or known activities or incidents of Hazing.

Please use this Title IX Report Form to report any incident or concern regarding sexual discrimination, sexual harassment, quid pro quo harassment, sexual assault, dating violence, dating violence, stalking, and/or retaliation.

Reporting Forms are not monitored 24 hours a day and do not notify first responders. If you need emergency assistance, please call 9-1-1 or the Jacksonville State University Police at (256) 782-5050.

There is no time limit for reporting violations of the Code of Student Organization Conduct; however, the longer someone waits to report an offense, the more challenging it may become for the Office of Community Standards to obtain information and witness statements, as well as to make determinations regarding alleged violations.

Though anonymous complaints are permitted, doing so may limit the University’s ability to investigate and respond to a complaint. Those who are aware of misconduct are encouraged to report it as quickly as possible to the Office of Community Standards and/or to the Department of Public Safety/University Police Department.
