OPEN ENROLLMENT DEADLINE REMINDER

The last day to enroll and/or submit changes to your PEEHIP Insurance, as well as the Humana/Compbenefits Vision plan is Friday, August 29, 2014.  Please remember that:

PEEHIP and Humana/CompBenefits Insurance Plans

• All forms mailed to PEEHIP must be postmarked no later than August 29, 2014.
• Members will be allowed to enroll and/or make changes to their PEEHIP Insurance plan ONLINE ONLY through SEPTEMBER 10.
• Enrollment forms for the Humana/Compbenefits Vision plan can be found on HR’s open enrollment page. Enrollment or changes to this plan should be submitted to Human Resources no later than August 29, 2014.  The Humana/Compbenefits forms should not be mailed to PEEHIP.
• The Humana/CompBenefit Vision Plan is completely separate from PEEHIP’s plan.  If you are currently enrolled in PEEHIP’s Vision plan and wish to change to Humana/CompBenefits you may do so by completing the appropriate enrollment form.
• It is your responsibility to cancel PEEHIP’s vision plan if you do not wish to continue participation.

Flexible Spending Plans

• The deadline for enrollment into PEEHIP’s Flexible Spending Plan is September 30, 2014.  The new flex plan year begins October 1, 2014.
• Enrollment in this plan is REQUIRED every year, regardless to whether or not you are making any changes.
