JSU Healthcheck

Before returning to campus, all students, faculty and staff must complete JSU Healthcheck, a COVID-19 assessment tool. This will help us all return to campus safely and help our community beat COVID-19.

Self Reporting Form

This form is for members of the JSU community to report directly to our Task Force that they tested positive for COVID-19. It is not for those that believe they were exposed.

COVID-19 Terms to Know

Quarantine - is known as the separation of a person or group of people reasonably believed to have been exposed to a person who has tested positive for COVID-19. In order to slow the spread of this virus here at Jacksonville State University, we ask that those who are notified by our Epidemiology Team and asked to quarantine that they do so for 14 days.

Isolation - is the term used once someone has tested positive for COVID-19. Those who test positive should fill out the Self-Reporting form linked on this slide. In order to limit exposure among people who are not infected, JSU is asking those who are in isolation to stay separated from others for 10 days from the date of testing, or if symptomatic, enter self-isolation for 10 days from the onset of symptoms.

Exposure - When you are told that you have been exposed, this means that you have been within 6 feet of an individual for more than fifteen minutes, with a confirmed case of COVID-19. If you have been exposed to the virus, a member of the JSU Epidemiology Team will contact you directly via phone or through your JSU email to discuss the next steps that you need to take.

Epidemiology Team (Epi Team) - this is a team of individuals here at Jacksonville State that assist with contact tracing once someone tests positive and fills out the Self-Reporting form on their MyJSU account. If you have been exposed to someone who has tested positive, this team will notify you via phone or by email on your JSU account.

Self-Reporting Form - This is an internal form for all JSU students, faculty, or staff members to be able to complete after they have tested positive for COVID-19. Once you have successfully completed the form, the Epi Team will reach out to you directly to assist with contact tracing.

JSU Healthcheck - this is a web-based assessment tool powered by GuideSafe that allows users to report their COVID-19 symptoms and exposure history. All JSU students, faculty, and staff are required to log in to the JSU Healthcheck website to complete a short assessment daily. Once completed, users will receive a response categorizing their risk as “Least Risk,” “May Be at Risk” or “Greater Risk” – each with its own instructions on how to proceed to campus for class or work.

JSU Student Health Center COVID-19 Test - COVID-19 testing is available to employees and students who are exhibiting symptoms at the JSU Student Health Center. The test conducted is a rapid test and utilizes the more invasive nasopharyngeal swab, but results are available within 15 minutes. If you are experiencing symptoms, we are encouraging you to report to the Student Health Center immediately for rapid testing.
