Flu Shots for Employees Rescheduled
Flu Shots have been rescheduled for Monday, November 5, 2007 from 1:30 p.m. – 3:30 p.m. at the TMB - Classroom 302.

There is no cost to you or your adult dependents if you have the PEEHIP medical coverage or one of the Southland Supplemental Plans. The vaccinations will be given on a first-come, first-serve basis, and employees will be required to show proof of insurance before the vaccination can be given (no exceptions will be made). 

In order to avoid long delays, we have attached for your convenience the Alabama Department of Public Health Vaccine Administration Form, which you may access here. Also, please remember to wear clothing that will expose your upper arm where the vaccination will be injected.
