Rescheduling of Flu Shots

FLU SHOTS HAVE BEEN RESCHEDULED

As you may have seen in recent news announcement the Health Department is facing a shortage of flu shots.  As a result of this shortage the Public Health Department has informed us that the vaccine will not be available for our scheduled session on Friday, November 17, 2006.

 

Flu Shots have been rescheduled for Monday, November 27, 2006 from 9 a.m. – 12 p.m. at the TMB Auditorium. The vaccinations will be given on a first-come, first-serve basis and employees will be required to show proof of insurance before the vaccination can be given (no exceptions will be made).  In order to avoid long delays, we have attached for your convenience the Alabama Department of Public Health Vaccine Administration Form, which you may access here. Also, please remember to wear clothing that will expose your upper arm where the vaccination will be injected.
