Application Contents:
There are 2 types of forms that can be filed for application into the International House Program. Please submit only one. 
Duplicate copies will be discarded unless any information needs to be updated.

(1)  Chose only one of the following formats:

        Application Format 1: a downloadable lined form (.doc format) that allows you to
        print and fill out using either a pen or typewriter.

        Application Format 2:  a downloadable form (.doc format) that you can fill out
        using a word processing program.  After completing the form, you can either
        e-mail (see instructions in packet) or print and mail (or fax) to the IHP.

(2)  In addition to one of the the above forms, the following form must be filed for application.

        Recommendation Form: The following recommendation form (3) must be included
        in your packet (see packet information on submission instructions).

To have a complete application, you must have and do the following:
Completed Application Form
Three (3) Recommendation Forms
Two (2) photos
       - an individual picture of you
       - a picture of your family
Have all forms (application and recommendations) turned into the International
       House no later than January 31 for each academic year.
