Memo To: Jacksonville State University Full-time Employees From: Office of Human Resources Re: CompBenefits Vision Plan The Open Enrollment period for the CompBenefits Vision Plan begins July 1st and ends August 31st for changes to be effective October 1st. Open Enrollment forms can be picked up in the Office of Human Resources. CompBenefits enrollment/change forms should be completed and returned to the Office of Human Resources (329 Bibb Graves Hall) no later than August 31st. If you do not wish to make changes to your CompBenefits Vision Plan coverage you need to do NOTHING. You will remain enrolled in the same or existing plan(s), and the appropriate premium will continue to be deducted. Employee rates are listed below. CompBenefits Single $7.44 CompBenefits Employee + 1 $14.86 CompBenefits Family $19.88 The CompBenefits Vision Plan is completely separate from the Vision Plan offered by PEEHIP Administered by Southland National. If you are currently enrolled in PEEHIP's plan and wish to change to CompBenefits you must complete an enrollment form for CompBenefits and most importantly you must remember to complete the appropriate form to cancel PEEHIP's Vision Plan. Failure to cancel PEEHIP's plan will result in you being and deducted for both plans until the next open enrollment period.
