Documentation for Certification List

Please include documentation for the following:

____ Copy of ATP membership certificate or cancelled check for membership.

____ Documentation of training, including the number of hours.

____ Provide the required letter (s) of recommendation.

____ Provide an overview of training program including materials where applicable and topics.

____ For all levels provide appropriate certificates or documentation of each level of training completed.

____ If applying for a TRAINER certificate, the documentation must include your presentation materials.

____ Application Form
