Make an appointment with an advisor in the Office of the Associate Vice President for Academic Affairs, Room 201 Bibb Graves Hall, 782-5004 or 5284. Your appointment should be made no later than three weeks prior to the semester for which you are petitioning to return.
Complete the following information and present these materials at the time of your appointment.
1. Prepare a letter requesting reinstatement: Address your
letter to the attention of Dr. Joe Delap, Associate Vice President for
Academic Affairs,
Room 201 Bibb Graves Hall, Jacksonville State University, Jacksonville,
Alabama
36265 or email to
jdelap@jsu.edu.
Your letter should be typed and should include the following:
< Name, Mailing Address, and Student Number;
< The semester you wish to return;
< The reason(s) you feel you deserve another opportunity;
< Your educational goals, your major and minor;
< Your plan to finance your education
(suspended/dismissed students are not eligible for financial aid during
their
reinstated semester, unless an appeal is made to the Financial Aid
Appeals
Committee).
2. Prepare a trial schedule and meet with your academic advisor for approval of the courses you wish to take. However, registration must occur in 201 Bibb Graves Hall.
3. If you have not been out of school for the required suspension/dismissal period (1 semester for suspension or 1 year for dismissal), you must also submit three letters of reference from faculty who can recommend that you be allowed to return to Jacksonville State University. These references must be from three JSU faculty members who are familiar with your goals.
4. Read the policy on reinstatement, on the reverse side of this page, and sign the attached statement of understanding. At your appointment with an advisor in Room 201 Bibb Graves Hall you will reach a specific agreement on how you can improve your academic performance.
REMEMBER TO BRING: 1)Letter Requesting Reinstatement, 2) Signed Policy on Reinstatement, 3) Letters of Reference if required and 4)Trial Schedule signed by your advisor.
Conditions for completing a reinstated term:
(1) That you make no less than a grade of "C" or better in all
attempted course
work;
(2) That you enroll in no more than twelve hours for Fall/Spring, six
hours
for summer terms of course work;
(3) Special Conditions: That you complete an academic progress
report
(in 201 Bibb Graves) every three weeks for Fall/Spring, every week for
summer terms.
(4) That you meet with your academic advisor for schedule approval
prior
to registering for classes.
(5) That your instructor(s) complete progress reports provided by this
office.
The administration at Jacksonville State University recognizes that extenuating circumstances may have precipitated a student's poor academic performance and unsatisfactory academic progress. Such circumstances are not excuses for poor academic progress but may justify consideration for a student’s reinstatement.
Students who are suspended or dismissed for academic reasons have the option of petitioning for reinstatement through the Associate Vice President for Academic Affairs. Reinstatement is for one semester only.
The reinstated student must maintain a 2.00 grade point average for each attempted course during the reinstated semester. Additional requirements specified by the Office of the Associate Vice President for Academic Affairs also must be met during the reinstated semester. Additional requirements include a course load of no more than the minimum full-time load for the semester of reinstatement, consultation with an academic advisor, and any special conditions necessary for the individual's reinstatement, e.g. specific courses or removal of incompletes.
The Associate Vice President for Academic Affairs maintains an active file on each reinstated student. The file documents are used to monitor academic advisement activities and academic progress for each reinstated student.
Students not maintaining the G.P.A. requirement or any condition of reinstatement will be suspended or dismissed from the University. The reinstated student in an academic suspension status must remain dropped for one semester. The reinstated student placed in an academic dismissal status must remain dismissed for one calendar year (12 months). No credit for the purpose of transfer may be earned from any other institution of higher education while a student is on “academic suspension” or “academic dismissal.” In addition, the student will be ineligible to receive financial aid during reinstatement, unless an appeal is made to the Financial Aid Appeals Committee.
